Cardinal Fitness is a gym and fitness center chain that operates four locations in the Chicago metro area. The gym focuses on being affordable, local and convenient for members. The membership may have worked well while you had it, but you’re moving out of the area or you’ve reached your fitness goals and taking a break for some time. Either way, the cancellation details are below.

To get started on cancelling your Cardinal Fitness account, you will need:

  • Address of your Cardinal Fitness location
  • Account number
  • Full name(s) on file
  • Address on file
  • Email on file
  • Reason for cancellation
  • Last 4 digits of the subscriber’s social security number
  • Last 4 digits of the subscriber’s driver’s license
  • Date of birth

Steps to Cancel

  1. Write a letter that includes the date, your name, identifying information like date of birth, driver’s license number, and the last four digits of account being billed. Make sure to include your billing address on the letter as well. Also include the address of the gym location where you signed up for membership, your signature, the date and membership number and client’s name. Explicitly state your desire to cancel and request that it be done effective immediately. If you are still within your contract (less than a year, or not month to month) you may need to show proof of a move or injury. Some gyms may allow a buyout of the contract. It is advised to call your gym ahead of time so that you know the terms of the agreement.
  1. Get address of location where contract was signed and send cancellation letter, via certified mail return receipt requested, to this location.
  1. Call the gym in question a week or two later to confirm that the membership was cancelled.

Insider Tip

Follow up with the gym, and keep an eye on your account statements to confirm no future billings take place.

Please note

A 30-day written notice is required to end the membership. You are still responsible for any charges that occur within the 30 days after your notice to cancel.

  1. Log onto http://www.melaleuca.com/youroptions.
  2. Print cancellation form (after working your way through several cancel prompts). Form can be faxed or mailed.
  3. Log in with your username/email and password. If you don’t have a web account, click the “Don’t have a web account” link and enter your member number, phone number (with area code) and zip or postal code. Continue through the prompts and then print your completed form.
  4. You can either fax or mail the form; all cancellation requests must include name, date, and signature.

A monthly fee covers your identity monitoring across certain channels. If you are replacing this service with some other form of monitoring or you’ve decided to stop monitoring your credit, the steps for canceling are below.

To get started on canceling your Identity Guard account, you will need:

  • Last 4 digits of the customer’s social security number
  • Date of birth
  • Customer’s name
  • Customer’s address

Steps to Cancel

  1. Send email with a signed cancellation letter, then call 1-800-452-2541. Simply call the phone number and speak with a representative. The representative will do his or her best to keep you as a customer, but if you are intent on canceling, stay resolute. They do offer pro-rated refunds. Be sure to ask for a refund and it will be issued within the next billing cycle. Make sure you have your social security number handy.

They have monthly and yearly subscriptions.

Amazon Prime is a paid membership to access extra benefits on an otherwise free online retail site. For example, Prime offers expedited shipping, free movies, music and books, and other benefits. However, you may not do enough online shopping to justify the membership. In that case, the cancellation steps for Amazon Prime are below.

To get started on cancelling your Amazon Prime account, you will need:

  • Email address of subscribing member
  • Password

Steps to Cancel

  1. Log into Amazon.com account with your email address and password.
  2. Click the “Your Account” option under your name at the top right.
  3. Choose the “Manage Your Prime Membership” option.
  4. Click “do not auto-renew” or “do not upgrade.” Choosing this option will ensure you receive Prime benefits for the remainder of your membership. At the end of your current period, your membership will expire and your card will not be charged.

Insider Tip

When signing up for a free trial, you may not purchase anything with the Prime account (utilizing the free two-day shipping). If you do, then you have committed to the Prime membership and cannot cancel or refund the $99.

Many users sign up for a “Free Trial” whereby they are charged $99 and if they decide to cancel, they can do so by clicking “cancel prime membership” rather than “do not auto-renew” and they will immediately be refunded $99.

You cannot refund the $99 prime membership if you have purchased anything through Prime.

  1. Call 1-800-870-6397 from Monday through Friday, 6 a.m. to 6 p.m., and request that your account be cancelled. Make sure to ask for a confirmation number, as some clients claim that they still receive cancelled subscriptions.
  1. You can also email your request to subscriberservices@mercurynews.com. Be sure to include the subscriber’s full name and mailing address or email address depending whether you signed up for home delivery.

The easiest method is by phone. There are many ways to control and modify your subscription on their customer care website. You will need to have a username and password to access the customer care online. You can also create an account, but if you’re just going to cancel the membership it’s easier to call.

The United Readers’ Service is a magazine service. The company, in operation since 1972, operates out of Ohio and is a provider of many different magazine titles. If your current magazine subscription (whether you ordered it yourself or received it through a gift) is through the United Readers’ Service, you will need to cancel with them – not your magazine itself. Steps to Cancel

  1. Call (888) 369-7323 between 9 a.m. and 5 p.m. EST.
  2. Speak to a representative and request to cancel. If your contract is still current, you may not be able to cancel your account. In this case, you still may be able to get out of the contract by disputing the charges with your bank.

Typically, this company will offer a discount or shorten the terms of the contract, instead of cancelling. It is usually possible to switch magazine subscriptions as well.

Ebay is the world’s online marketplace; a place for buyers and sellers to come together and trade almost anything. If you’ve made a purchase on ebay and you need to cancel it for some reason, see the instructions below.

To get started on cancelling an ebay transaction, you will need your:

  • Email address associated with your ebay account
  • User ID for ebay
  • Password for ebay

Steps to cancel
Log onto your ebay account. Go to Purchases. Select the purchase you wish to cancel and select cancel.

Insider Tip
Cancel the purchase as quickly as possible. If you cancel the purchase before it has been shipped, it is a very simple one-click process. If the purchase has already been shipped, you will be responsible for returning the purchase. You will be charged and then credited for the purchase once it is returned. You may also be responsible for the return shipping fees.

Please note

If the order has already been shipped, go to the Customer Support/Resolution Center, open a new case. Select the correct purchase. In the box provided, state that you no longer want the purchase, the purchase was made in error. Submit. You will receive a reply from the seller with instructions on how to return the purchase.
